Community Health Workers
Our community health workers are employees of AmeriCorps and serve as a valuable resource to our patients.
Our two CHWs work to address our patients’ Social determinants of health (SDOH) - the nonmedical factors that influence health outcomes.
Abbie and Zahory work closely with all of our partners across the community and truly assist in serving the whole person.
