Community Health Worker Role
Zahory is our Community Health Worker who interacts with our patients and provides additional "non-medical" assistance and referrals. Our Community Health Workers work hard to determine other social determinants of health that may be working as a barrier to healthy living for our patients (ex: access to housing, food, legal assistance, transportation, etc). She also serves as a medical interpreter.
